Sec. 1. (a) An owner or operator shall obtain written authorization from the division of reclamation under this rule to operate a Class II well.

(b) Written authorization to inject shall be issued after the owner or operator has submitted to the division of reclamation the following:

1. (1) A completion report showing the well has been constructed as a Class II well in accordance with the permit issued under 312 IAC 29-5.
2. (2) An analysis of the injection fluid in accordance with section 7 of this rule.

3. (3) Certification that the well meets the requirements for external mechanical integrity under section 4 of this rule, verified by either of the following:

   1. (A) Cement tickets.
   2. (B) Other measures specified in section 4(c)(1) through 4(c)(6) of this rule.
4. (4) Certification that the well has passed an internal mechanical integrity test in accordance with section 2 of this rule.

(c) The division director may require additional testing or special equipment if appropriate to the protection of an underground source of drinking water.
