Sec. 3. (a) In addition to the reporting requirements for spills in section 2 of this rule, an owner or operator shall report all fires or any blowout or other incident involving the loss of control of a well or any facility associated with the drilling operation or plugging of wells for oil and gas purposes to the division of reclamation within two (2) hours of discovery by telephone to the inspector, and by e-mail to spill@dnr.in.gov.
(b) The report shall include the following information:
