Sec. 12. (a) A permit application to drill a test hole or holes shall be made on a form prescribed by the division.

(b) A permit application shall be signed by the person designated as the operator on the application or by an authorized agent. Upon a request by the division, a person who signs as an agent for the operator must furnish satisfactory evidence of authority.

(c) An applicant shall remit with the application a permit fee of one hundred dollars ($100) in cash or by check or draft, payable to the department.
