Sec. 64. Written policy, procedure, and practice provide that records in the facility case files are:

1. (1) clearly identified with a title or label;
2. (2) completed with data verified with a dated, staff signature by the recording or inputting staff; and
3. (3) marked confidential or maintained in a secure way that ensures confidentiality.
