1. (1) The applicant is a veteran and wishes to have an indication of the applicant's veteran status appear on the driver's license or permit.
2. (2) The applicant has a medical condition of note and wishes to have an identifying symbol and a brief description of the medical condition appear on the driver's license or permit.

3. (b) The bureau shall inform an applicant that submission of information under this section is voluntary.
