Ind. Code § 9-18.5-8-5

Temporary parking placard

  1. (a) The bureau shall issue a temporary placard to an individual who is certified by:

    1. (1) a health care provider listed in section 4(b) of this chapter as having:

      1. (A) a temporary physical disability that requires the temporary use of a wheelchair, a walker, braces, or crutches;
      2. (B) temporarily lost the use of one (1) or both legs; or
      3. (C) a temporary and severe restriction in mobility due to a pulmonary or cardiovascular disability, an arthritic condition, or an orthopedic or neurological impairment; or
    2. (2) an optometrist or ophthalmologist who has a valid unrestricted license to practice optometry or ophthalmology in Indiana to be temporarily:

      1. (A) blind (as defined in IC 12-7-2.1-38 (2)); or
      2. (B) visually impaired (as defined in IC 12-7-2.1-349 ).
  2. (b) A certification under this section must:

    1. (1) be in a manner and form prescribed by the bureau; and
    2. (2) state the expected duration, including an end date, of the condition on which the certification is based.
  3. (c) A temporary placard issued under this section expires on the earlier of the following:

    1. (1) One (1) year after the date on which the placard is issued.
    2. (2) The end date set forth in the certification under subsection (b).
