The corporation shall submit an annual report to the budget committee and the legislative council before September 1 of each year. The report must be in an electronic format under IC 5-14-6 and must contain the following information concerning fund activity in the preceding state fiscal year:

1. (1) The name of each entity receiving a grant from the fund.

2. (2) The location of each entity sorted by:

   1. (A) county, in the case of an entity located in Indiana; or
   2. (B) state, in the case of an entity located outside Indiana.
3. (3) The amount of each grant awarded to each entity.
