The registry must contain:

1. (1) the name;
2. (2) the age;
3. (3) the last known city of residence;
4. (4) a photograph, if available;
5. (5) a description of the crime of child abuse conviction; and

6. (6) any other identifying information, as determined by the division;

   of every person convicted of a crime of child abuse.
