1. (a) An entity may at any time during which a certificate of qualification is in effect demand a new statement. If a contractor does not provide a new statement not later than sixty (60) days after the request is made, the entity may void the contractor's certificate of qualification.
2. (b) If a contractor fails to provide complete and true information in an application, the application for prequalification shall be rejected.
