Ind. Code § 35-40-12-4

Name and address of person to receive notice

  1. (a) This section applies if the victim is an entity other than an individual.
  2. (b) A request for notice under IC 35-40-10 must identify the name, electronic mail address, and mailing address of the person who is to receive notices and consultations on behalf of the entity.
