The clerk of the circuit court shall furnish a statement with each remittance of change of venue costs. Statements described in this section must:

1. (1) show in detail the cause number in the county to which remittance is made, title of case, and the items of costs paid; and

2. (2) be made on forms prescribed by the state board of accounts.
