"Electronic seal" means information that:

1. (1) is specific to an individual notary public;
2. (2) is attached to or associated with a notarized electronic record; and

3. (3) contains:

   1. (A) the words, "notary public";
   2. (B) the words, "state of Indiana";
   3. (C) the notary public's name as it appears on the notary public's commission certificate;
   4. (D) the words, "commission number", followed by the commission number of the notary public; and
   5. (E) the words, "my commission expires", followed by the expiration date of the notary public's commission.
