1. (a) If medical care is provided to a child who receives foster care, the person who has custody of the child shall inform the provider that the provider is required to file a copy of:

   1. (1) the form provided under IC 31-28-3 ; and

   2. (2) the child's medical treatment record for the medical care;

      with the local office in which the child resides.

2. (b) The provider shall file the form and record with the local office.
