The state department of health shall:

1. (1) prescribe a registration form for the information that a putative father submits under section 9 of this chapter; and

2. (2) make the registration forms available through:

   1. (A) the state department;
   2. (B) each clerk of a circuit court; and

   3. (C) each local health department.
