The following persons shall send a copy of a contact preference form received by the person from a birth parent to the state registrar:

1. (1) The department.
2. (2) A local office.
3. (3) A licensed child placing agency.
4. (4) A professional health care provider.
5. (5) An attorney.
6. (6) A court.
