Ind. Code § 31-19-21-6

Storage and indexing of consents

The following persons shall provide for the storage and indexing of consents made under this chapter to carry out IC 31-19-24 through IC 31-19-25.5 :

  1. (1) The state registrar.
  2. (2) The department.
  3. (3) County offices of family and children.
  4. (4) Licensed child placing agencies.
  5. (5) Professional health care providers (as defined in IC 34-6-2.1-165 ).

  6. (6) Courts.
