As used in this chapter, "personal insurance policy" means a policy that:

1. (1) provides one (1) or more of the kinds of insurance described in Class 2 or Class 3 of IC 27-1-5-1 ; and
2. (2) is underwritten on an individual basis for personal, family, or household use.
