1. (a) This section is effective beginning July 1, 2021.

2. (b) Except as provided in subsection (c), a:

   1. (1) facility; and

   2. (2) practitioner;

      shall provide to a covered individual, at least five (5) days before a health care service is scheduled to be provided by the facility or practitioner to the covered individual, a good faith estimate of the amount that the facility or practitioner intends to charge for each health care service to be provided to the covered individual and in compliance with IC 27-1-46-11 (c).

3. (c) A facility or a practitioner is not required to provide the good faith estimate required in subsection (b) if the health care service to be provided to the covered individual is scheduled to be performed within five (5) business days after the health care service is ordered.
