As used in this chapter, "utilization review entity" means an individual or entity that performs prior authorization for one (1) or more of the following:

1. (1) An employer who employs a covered individual.
2. (2) A health plan.
3. (3) A preferred provider organization.

4. (4) Any other individual or entity that:

   1. (A) provides;
   2. (B) offers to provide; or

   3. (C) administers;

      hospital, outpatient, medical, prescription drug, or other health benefits to a covered individual.
