As used in this chapter, "cost sharing" means any copayment, coinsurance, deductible, or other similar charge that is:

1. (1) required of a covered individual for a health care service covered by a health plan, including a prescription drug; and

2. (2) paid:

   1. (A) by; or

   2. (B) on behalf of;

      the covered individual.
