As used in this chapter, "pharmacy affiliate" means a pharmacy, including a specialty pharmacy, that directly or indirectly, through one (1) or more intermediaries:

1. (1) owns or controls;
2. (2) is owned or controlled by; or

3. (3) is under common ownership or common control with;

   an insurer, a pharmacy benefit manager, or other administrator of pharmacy benefits.
