(a) Each applicant for a license as a physical therapist or certification as a physical therapist assistant must present satisfactory evidence that the applicant:

1. (1) does not have a conviction for a crime that has a direct bearing on the applicant's ability to practice competently; and
2. (2) has not been the subject of a disciplinary action initiated by the licensing agency of another state or jurisdiction on the grounds that the applicant was unable to practice as a physical therapist or physical therapist assistant without endangering the public.

(b) An applicant may appeal the board's decision to deny licensure or certification to the board within fifteen (15) days after the applicant receives notification of the board's decision. Upon receiving an appeal under this subsection, the board shall set the matter for an administrative hearing under IC 4-21.5 .
