Ind. Code § 25-23-1-19.7
(a) This subsection applies to an applicant for renewal who has never received a renewal of prescriptive authority under section 19.5 of this chapter and whose prescriptive authority has never lapsed. If the applicant was initially granted prescriptive authority:
(2) at least twelve (12) months before the expiration date of the prescriptive authority, the applicant shall, subject to IC 25-1-4-3 , attest to the board that the applicant has successfully completed at least fifteen (15) contact hours of continuing education. The hours must:
