Upon receipt of satisfactory evidence from a licensed health facility administrator or licensed residential care administrator that the administrator's license has been:

1. (1) lost;
2. (2) stolen;
3. (3) mutilated; or

4. (4) destroyed;

   the board shall issue a duplicate license to the administrator.
