To qualify for a license under this article, an individual must do the following:
- (1) Satisfy the requirements of IC 25-14.5-3 .
- (2) Satisfactorily complete an application for licensure, furnished by the board, in accordance with the rules adopted by the board.
- (3) Pay the application, examination, and licensure fees established by the board.
- (4) Except to the extent that section 4 of this chapter applies, successfully pass the qualifying examination adopted by the board as described in IC 25-14.5-5 .
