1. (a) As used in this chapter, "practitioner" means an individual or entity duly licensed or legally authorized to provide health care services.

2. (b) The term does not include the following:

   1. (1) A dentist licensed under IC 25-14 .
   2. (2) An optometrist licensed under IC 25-24 .
