As used in this chapter, "good faith estimate" means a reasonable estimate of the price a practitioner anticipates charging for an episode of care for nonemergency health care services that:

1. (1) is made by a practitioner under this chapter upon the request of:

   1. (A) the individual for whom the nonemergency health care service has been ordered; or
   2. (B) the provider facility in which the nonemergency health care service will be provided; and
2. (2) is not binding upon the practitioner.
