Before receiving a scholarship under this chapter, the applicant must enter into a contract with the commission agreeing to:

1. (1) the terms and conditions described in section 5(a)(5), 5.5(b)(4), or 5.5(c)(4) of this chapter, as applicable; and
2. (2) any other terms and conditions established by the commission.
