1. (a) An employment certificate shall be issued:

   1. (1) in a form approved by; and

   2. (2) under rules adopted under IC 4-22-2 by;

      the department of labor and the state board.

2. (b) The style of the form and the rules adopted under this section must:

   1. (1) be consistent with this chapter; and

   2. (2) promote uniformity and efficiency in the administration of this chapter.
