1. (a) In grade 6, a student and the student's parent shall develop an initial graduation plan. The plan must include the following:

   1. (1) A statement of intent to graduate from high school.

   2. (2) An acknowledgment of the importance of:

      1. (A) good citizenship;
      2. (B) school attendance; and
      3. (C) diligent study habits.
2. (b) The plan must become part of the student's permanent school record.
