1. (a) Beginning in the 2022-2023 school year, each school corporation and charter high school shall provide:

   1. (1) each student; and

   2. (2) a parent of each student;

      of a school corporation or a charter high school who is in grade 12 with a notice regarding the information described in IC 21-18-6-6 (b).

2. (b) Each school corporation and charter high school shall use the model notice prepared by the commission for higher education under IC 21-18-6-6 .

3. (c) A nonpublic high school may provide:

   1. (1) a student; or

   2. (2) the parent of a student;

      of a nonpublic school who is in grade 12 with a notice regarding the information described in IC 21-18-6-6 (b).
