A member of the statewide maternal mortality review committee shall:

1. (1) sign a confidentiality form prepared by the statewide maternal mortality review coordinator under IC 16-50-2-3 ;
2. (2) review the purpose and goals of the statewide maternal mortality review committee; and
3. (3) review the data collection form developed by the statewide maternal mortality review coordinator under IC 16-50-2-2 .
