Ind. Code § 13-18-16-7
A person responsible for the operation of public water systems shall submit:
(2) reports of operation pertaining to the sanitary quality, chemical quality, or adequacy of water supplied by those systems;
that the commissioner requests. The operator certified under IC 13-18-11 must verify under oath the reports of operation.
