An agency or a political subdivision required to verify the eligibility of an individual under section 5 of this chapter shall:

1. (1) require the individual to execute a verification stating under penalty of perjury that the individual is a:

   1. (A) United States citizen; or
   2. (B) qualified alien (as defined under 8 U.S.C. 1641); and
2. (2) maintain a verification executed in accordance with subdivision (1) for at least five (5) years.
