When assistance is granted to a dependent child under IC 12-14-1 through IC 12-14-8 , the award made must be entered on a written notice prescribed by the division that designates the following:

1. (1) The name and residence of the recipient.
2. (2) The amount of the award.
3. (3) The date when the assistance is to begin.

4. (4) Any other fact required by the division.
