Ind. Code § 10-14-4-9
Grant application for eligible entity other than individual

To qualify for a grant under section 6 of this chapter, the eligible entity must apply to the agency on forms provided by the agency. The application must include the following:
- (1) A description and estimated cost of the damage caused by the disaster to the entity.
- (2) The manner in which the entity intends to use the grant money.
(3) Any other information required by the agency.
