This chapter does not apply to any of the following:

1. 1. A policy of credit accident and health or credit accident and sickness insurance.
2. 2. A policy of accident and sickness insurance which is exempt from the provisions of sections 514A.1 to 514A.12 by virtue of an exemption set forth in section 514A.1 or 514A.8.
3. 3. Any evidence of coverage issued to an enrollee of a health maintenance organization under chapter 514B.
