The commission shall prepare and issue any permit granted in accordance with section 479.12. Said permit shall show the name and address of the pipeline company to which it is issued and identify by reference thereto the decision and order of the commission under which said permit is issued. It shall be signed by the chairperson of the commission and the official seal of the commission shall be affixed thereto.
