A parent may request a cost-of-living alteration by submitting all of the following to child support services:

1. 1. A written request for a cost-of-living alteration to the support order signed by the parent making the request.
2. 2. A statement signed by the nonrequesting parent agreeing to the cost-of-living alteration to the support order.
3. 3. A statement signed by each parent waiving that parent’s right to personal service and accepting service by regular mail.
4. 4. Other documentation specified by rule of the department.
