1. 1. A pharmacy shall report in writing to the board, pursuant to its rules, the following:

   1. a. Permanent closing.
   2. b. Change of ownership.
   3. c. Change of location.
   4. d. Change of pharmacist in charge.
   5. e. The sale or transfer of prescription drugs, including controlled substances, on the permanent closing or change of ownership of the pharmacy.
   6. f. Change of legal name or doing-business-as name.
   7. g. Theft or significant loss of any controlled substance on discovery of the theft or loss.
   8. h. Disasters, accidents, and emergencies that may affect the strength, purity, or labeling of drugs, medications, devices, or other materials used in the diagnosis or the treatment of injury, illness, and disease.
2. 2. A pharmacist shall report in writing to the board within ten days a change of name, address, or place of employment.

3. 3. A wholesaler shall report in writing to the board, pursuant to its rules, the following:

   1. a. Permanent closing or discontinuation of wholesale distributions into this state.
   2. b. Change of ownership.
   3. c. Change of location.
   4. d. Change of the wholesaler’s responsible individual.
   5. e. Change of legal name or doing-business-as name.
   6. f. Theft or significant loss of any controlled substance on discovery of the theft or loss.
   7. g. Disasters, accidents, and emergencies that may affect the strength, purity, or labeling of drugs, medications, devices, or other materials used in the diagnosis or the treatment of injury, illness, and disease.
   8. h. Other information or activities as required by rule.
