Rule 546-2-.02. Reinstatement

1. 1) An applicant for reinstatement of a revoked or lapsed license must:

   1. a) Submit a completed application on the approved form
   2. b) Pay the reinstatement fee dictated by the fee schedule
   3. c) Submit proof of a current surety bond; and
   4. d) If the applicant is not a citizen, the applicant must submit an updated citizenship affidavit and secure and verifiable document.
