Rule 480-30-.03. Labeling

All drugs dispensed in accordance with a dispensing procedure and under authority of a job description or nurse protocol must be labeled with the following information:

1. (a) Date and identifying serial number;
2. (b) Name of patient;
3. (c) Name of practitioner prescribing;
4. (d) The name, address and telephone number of the facility where the drugs are dispensed in accordance with a dispensing procedure and under the authority of a job description or nurse protocol;
5. (e) Name of drug and strength;
6. (f) Directions for use to the patient;
7. (g) The expiration date of the drug; and
8. (h) Any information required by the Drug Enforcement Administration or the Food and Drug Administration.
