Rule 480-22-.10. Labeling of Controlled Substance Prescription Drug Orders

1. (1) A pharmacist filling a prescription drug order for a C-II, III, IV or V substance shall affix to the package a label showing the following:

   1. (a) The name, address and telephone number of the pharmacy;
   2. (b) The prescription drug order serial number;
   3. (c) The date the prescription was initially filled or refilled;
   4. (d) The name of the patient;
   5. (e) The name of the prescribing practitioner;
   6. (f) The directions for use;
   7. (g) The expiration date of the dispensed drug; and
   8. (h) Cautionary statements, if any, as required.
2. (2) All prescription drug orders for C-II, III, IV or V controlled substances shall be kept in accordance with this chapter.
