The required fee must accompany the appropriate application as noted in the following schedule:

1. (a) Application fee -- As set forth on the fee schedule adopted by the Board;
2. (b) Examination fee -- As set forth on the fee schedule adopted by the Board;
3. (c) Re-Examination fee -- As set forth on the fee schedule adopted by the Board;
4. (d) Examination Review fee -- As set forth on the fee schedule adopted by the Board;
5. (e) Biennial Renewal fee -- As set forth on the fee schedule adopted by the Board;

6. (f) Delinquent Renewal fee -- As set forth on the fee schedule adopted by the Board;

   1. 1. All renewals after December 31st of the renewal year must be accompanied by the delinquent fee plus the renewal fee.

7. (g) Duplicate Certificate fee -- As set forth on the fee schedule adopted by the Board;

   1. 1. A duplicate certificate will only be issued as a replacement of an original or as a result of a legal name change.
8. (h) Endorsement License fee -- As set forth on the fee schedule adopted by the Board; and
9. (i) Federal Service Endorsement License fee -- As set forth on the fee schedule adopted by the Board.
