Rule 391-4-14-.01. Application for Aquaculture Registration

Each applicant for an aquaculture registration shall furnish the following information on their application, in addition to any information required by any other law, rule, or regulation:

1. (a) Applicants name, mailing address, and phone number;
2. (b) Name and kinds (fish hatchery, catch-out pond, wholesale fish operation, retail fish operation, or other type) of each business, corporation, agency or institution to be covered by the registration;
3. (c) The address and location, including county, where each facility is located; and
4. (d) The species of fish that will be included under the registration.
