Rule 351-8-.04. Complaints
Complaints submitted to the Commission shall include the following information: (a) The name and contact information of the complainant; (b) The name and license number, if applicable, of the person or entity that is the subject of the complaint; (c) A detailed description of the alleged violation of law(s) or rule(s); (d) The date of the alleged violation or, if such date is unknown, the date that the alleged violation was identified or became known by the complainant; (e) The address or description of the location where the alleged violation occurred; and (f) Information, documentation, or evidence to support the complaint.
