Rule 120-2-82-.05. Pharmacologic Agent Coverage Defined
Health insurance policies and plans, except limited benefit insurance policies, shall provide coverage for the following pharmacologic agents when prescribed by a physician licensed to practice medicine pursuant to Title 43: (1) Insulin of each class approved by the federal Food and Drug Administration (FDA) including formulations available either in a vial or cartridge; (2) Prescription insulin of each class approved by the federal Food and Drug Administration (FDA) including formulations available either in a vial or cartridge formulation; (3) Prescription oral medications of each class approved by the federal Food and Drug Adminstration (FDA) for the management of diabetes; (4) Oral products approved by the federal Food and Drug Administration (FDA) for the management of diabetes; (5) Glucagon kits; (6) Pharmacologic agents approved by the federal Food and Drug Administration (FDA) for the management of diabetes and its complications.
