Rule 120-2-62-.05. Required Filings

The following items shall be submitted to the Commissioner of Insurance:

1. (a) A completed Form GID-67, entitled the "Application for Approval of Assumption Reinsurance Agreement," attached hereto and incorporated herein as "EXHIBIT C"; and
2. (b) A copy of the proposed notices of transfer which comply with the requirements of Rule 120-2-62-.04 of this regulation and bear unique form numbers.
