Rule 120-2-45-.04. Application for Certificate of Authority; Renewal

1. (1) Each application for a certificate of authority shall be made on the appropriate form prescribed by the Commissioner and found on the Commissioner's website. It shall be accompanied by all required documents. The application and related documents are subject to review and written approval by the Commissioner.
2. (2) A company holding an active certificate of authority shall renew its certificate of authority by paying the required fee.
