Rule 120-2-113-.04. Reporting Requirements

By July 1, 2026, and annually thereafter on or before January 1 of each calendar year, each insurer that utilizes prior authorization requirements shall submit to the Commissioner a filing concerning their respective program. Such filing shall include:

1. (1) A full narrative description of the program;
2. (2) The criteria for participation in the program;
3. (3) A list of the procedures and services subject to the program; and
4. (4) The number of healthcare providers participating in the program.
